Pelvic Pain and Interstitial Cystitis

If you have been suffering from pelvic pain or recurrent bladder infections you may have a more serious condition called interstitial cystitis (IC). Symptoms of IC sometimes mimic bladder infections, which can make it difficult to diagnose.

PELVIC FLOOR DYSFUNCTION

Pelvic Floor Dysfunction refers to a wide range of symptoms that occur when the muscles of the pelvis which surround the urethra, bladder and rectum are tender, spastic and hyperactive. This is thought to result from chronic inflammation of pelvic organs such as the bladder as in interstitial cystitis and chronic urinary tract infection, prostate inflammation as seen in prostatitis, or chronic vaginal infections with yeast or bacteria. The chronic irritation inherent in these inflammatory conditions can all eventually up-regulate the nerve supply to these pelvic organs, resulting in increased spasticity and hyperactivation of pelvic muscle groups which share the same nerve roots at the level of the sacral spinal cord. In addition, other pelvic structures are adversely effected by this neuropathic pelvic pain syndrome, resulting in the symptoms of vulvodynia- vaginal pain and burning, urethral and penile sensitivity, urinary frequency and urgency, as well as irritable bowel symptoms.

PELVIC FLOOR REHABILITATION

uses "hands-on" and "internal" physical therapy to relax pelvic muscle trigger points, release abnormal myofascial attachments and provide deep tissue massage of these muscle groups in spasm. Biofeedback and electrical stimulation is often used as part of the "hands-on" technique to facilitate recovery. Neuromuscular reeducation is critical for the return of normal functional activities of daily living, relief of pelvic pain, and return of normal urinary and bowel function. This physical therapy is performed only by specially trained Physical Therapists and usually requires visits to the Physical Therapy office one or two times a week for at least two months.
